ID Card Application Form

To request a ID card for access to ERC data viewing areas, please follow the instructions below:

  1. Download “ID Card Application Form” (PDF) file to your computer.
  2. Open the saved file (the Adobe Acrobat Reader is available for viewing .pdf files).
  3. Complete and save the form.
  4. Upload the complete form AND a copy of your official photo ID (Driver’s License or Passport) via the “Submit ID Card Application Form” widget below. Please do NOT email the complete form or your photo ID to ERC, as it contains secure private information.
